challenges: overcorrection, ectropion (eyelid eversion), deep creases, and wide double eyelids. For cases like this, it is rare to achieve a perfect result in just one operation. In this patient’s case, there are still some concerns remaining—such as a bifurcated crease at the inner corner of the left eye (right side in the photo), some residual ectropion, and a slight asymmetry in the crease width. We plan to perform a minor adjustment in the next session. While this first surgery brought us relatively close to the ideal result, it doesn’t always go this way. Unfortunately, initial surgeries can sometimes be unsuccessful and leave patients feeling troubled. That’s the nature of revision surgery. (That said, sometimes even first-time surgeries can go very well—though the chances of that are generally lower compared to revision cases.) We also adjust the shape of the eyelid fold. This patient had previously undergone double eyelid surgery at another clinic. While they did have a crease, it was: “Wider at the inner corner (medial canthus) and narrower at the outer corner (lateral canthus).” This can lead to an unnatural appearance or make the eyes look older. So we made the following corrections: 1. Narrowed the inner corner (using a lifting technique) 2. Widened the outer corner Since the right eyelid was thicker, we also performed ROOF (retro-orbicularis oculi fat) excision on the right side only. The result turned out beautifully, enhancing a more youthful appearance. There is still a slight asymmetry in the crease width from the center to the outer corner of the right eye. The patient previously underwent a double eyelid incision procedure at another clinic. Because the crease was set too wide, the eyelids became difficult to open, resulting in a condition resembling ptosis.
